We offer two efficient ways to register as a new patient for our healthcare services:
1. Self-Referral (No Physician Referral Needed):
For faster registration, please complete our online forms through our PATIENT PORTAL. Alternatively, you can download and print the forms from the FORMS tab. While online registration is preferred for a smoother intake process, we understand some patients may favor paper forms. If you'd like us to mail you a copy, please call or email our office.
2. Physician Referral:
If your physician is referring you, here's what to expect during the patient registration process:
We'll review the referral from your doctor. We'll create your account and email you a link to our patient portal, where we may request additional information. Depending on your appointment type, we'll either send you forms to complete through the portal or directly to your email. Once you've completed the forms, we'll review them and schedule your appointment with the appropriate provider based on insurance, specialty, and level of care.
During the registration process, we may contact you via text or email to request additional information (e.g., insurance card, email address). Our goal is to ensure your information is accurate and complete, allowing you to focus on your health.
